I wouldn't put it all down to luck. Human and/or equipment failure would surely play a significant part.

Luck, human error, equipment failure and Russian SAM's all factor in to Malaysian/ Indonesian bases airlines run of incidents. I did read that for a period of time the EU banned Indonesian based airlines for safety concerns.
Seperately, Boeing and presumably Airbus offer a satellite tracking system. Perhaps the airlines should buy into that sort of thing, considering massive gaps in radar coverage.
