The story is Kurt sang it wrong on the demo (started singing too early), but the band liked it, so kept it. Butch just takes credit for it.
The original demo sat around for at least 2 years before the "Nevermind" version was even recorded, so if it really was a mistake initially, it's likely that it just "became" part of the song through the passage of time. Vig clearly doesn't know this.

I just listened to the Unplugged version and he totally does the "Polly said" early there, too. So I'm inclined to believe ya'll.

yeah, it became part of the song, as if it was intentional the entire time. he sings it that way in all the live versions, and even the "New wave" version from Incesticide, which was recorded a year and a half after Nevermind.

sidenote, i had no idea that the song was originally named "Hitchhiker" (then "Cracker", which i knew) until now. Another wrinkle in my brain!

He did that on every single version ever recorded...years before and years after the Nevermind version. So it's not really a matter of debate either way.

Fun fact: he was inspired to do that by the false start at the end of Girls Just Wanna Have Fun.

I've had the same thought, particularly during live jams. So many end with what sounds like the band going "uhh I guess this is a good spot to end on"
i'd say worst fadeouts in music history would be a good new thread, but the answer is obviously hendrix's little wing.

The only fadeout I’ve ever had a problem with is “In the Air Tonight”

I do believe this is an accident wonderfully left in.

I believe Mike McCready fucks this up though i could be mistaken.

Around the 2:30 mark
On the long plain
See the rider in the night
See the chieftain
See the braves in cool moonlight
Who will love them
When they take another life
Who will hold them
When they tremble for the knife

Prior to Chieftan you hear someone hit a chord and slide up to the property chord....I believe that is a legit fuck up and corrected just barely in time.

The song should be like five minutes longer!

Unless I'm going crazy, he didn't do it on this one: https://www.youtube.com/watch?v=3QdsPZ6w2g4

Maybe that's what Vig had heard prior to his involvement.

During the opening clatter of seemingly random drum beats, horn stabs, and guitar hits in Evangelista's "Hello, Voyager!", at about 1:10, one can hear Carla Bozulich start saying "ow, ow, ow," because she hit herself on the hand when playing a tiny handheld drum.

Around about 2:40 into "Friends Don't Owe"by Vennart, Mike Vennart gives a little snorting chuckle that sounds like a cough when singing the lyric "Friends don't know what they have started/no more bursaries for old adversaries." Apparently the lyrics to the song are quite personal, and that particular line strikes Mike as being rather funny. You'll have to click through to the track on Bandcamp due to the way the code works, either Bandcamp or the board's. Can't find it on Youtube, though it could be found on Spotify if so inclined.

At 2:15 in Adam Ant's "Goody Two Shoes", there's either a very awkward edit or an error in the drumming. I can't really tell.

Dammit. I'm a huge AdamAnt fan and I fear I'll never be able to not hear that.
Think I’m going to try being kind to everyone a chance.

The pigeon heard during the main phrase is part of the beauty of the piece. Percussive found sound.
