Did they pick a model with a mole or was that just coincidence?
Did they have to ask a bunch of models if they had moles and where they were?
If it was a coincidence, did they talk about replacing her or photoshopping it out?
Did they choose a model and then add a mole for the cover?
Is it a physical addition or digital?
Is the mole a statement? Does it releate to the music on the album?

I've liked most QOTSA album covers.
Junip - Fields is very nice.
Pearl Jam's covers have been great with my least favorites being Ten & Avocado (though the zombie stuff in the booklet was fun).
