We’ve known for quite a while that COVID infections have long-term cardiac consequences, but this new study greatly expands our understanding of that topic. The researchers used a very large data set to compare rates among people who had and had not been infected with COVID, as well as looking at relationships with the severity of COVID (based on level of care received: not hospitalized, hospitalized, or admitted to the ICU). To be 100% clear, this study looked at COVID survivors. In other words, it looked at rates of cardiac problems after people cleared the initial COVID infections. Thus, these injuries or deaths are not included in the standard COVID stats. These are long-term consequences. There are several key findings that I will highlight below.

1. COVID results in a wide range of long-term cardiac problems. There are very serious long-term consequences of this disease. Across all groups, for example, COVID infections result in roughly 1 extra case of cardiac failure for every 100 individuals who are infected!

2. Rates of these issues increase with the severity of COVID, but the rates among non-hospitalized COVID patients were still significantly higher than the rates among the controls (people who did not catch COVID). In other words, even COVID cases that are not life-threatening themselves can cause future cardiac problems that can be life threatening. For example, even among those who weren’t hospitalized, COVID resulted in a 37% increase in risk of heart failure in the months following recovery from the initial infection (compared to the controls).

3. COVID resulted in increased risks of myocarditis and pericarditis even after accounting for potential side-effects of vaccines.

These data lead to several important conclusions.

1. The risk of COVID is far greater than simply dying of the initial infection. The true mortality rate is higher than the rate from just the initial infections, and even if you don’t die, you are at an increased risk of many serious heart and vascular issues, even if you only had a “mild” case of COVID.

2. No data on the vaccines has suggested that they cause rates of cardiac problems anything even close to the rates caused by COVID itself. Indeed, even for myocarditis and pericarditis, which vaccines likely, bur rarely, cause, your risk goes up substantially with COVID.

3. Based on all of this, good risk assessment clearly shows that getting vaccinated is the safter option (unless you are in the subset of people who have legitimate, diagnosed, medical conditions that prevent you from receiving vaccines). There is no reason to suspect that there will be future long-term consequences of the vaccines, but we know that COVID has serious long-term consequences.

Notes on reading the graph:

The first column of graphs (HR) shows the hazard ratios. Read these as the risk relative to the control group, where 1 = no change in risk, a number < 1 = a decreased risk, and a number > 1 = an increased risk. To translate these changes in risk to percentages (we are looking at relative risk here), take the difference from 1 and multiple by 100. Thus, an HR of 2 = 100% increase in risk, or a doubling of risk. HR of 0.5 = a 50% decrease in risk. Error bars are 95% confidence intervals. In other words, we are 95% sure that the true change in risk lies within those bars. If the bars cross the 1 line (in either direction) treat that as a non-significant result (i.e., a change of that magnitude is likely to occur even if there is no effect of COVID either way).

The second column shows excess numbers of cases per 1,000 people. In other words, these are additional cases of the given event caused by COVID (i.e., for every 1,000 people who survive COVID, X number will develop the given condition beyond the usual background rate). So read these as additional people that have this condition because of COVID.
