In 2000, the Niners cheated the cap and were fined $300K and lost two draft picks.
In 2012, Seattle cheated by having live contact drills during OTAs and were penalized by having two practices cancelled by the NFL. They've also had a half dozen players suspended for PED use recently.

New England was punished for having a camera in the wrong place during live NFL action, there's nothing about taping other team's practices. I imagine that many teams - based on what Jimmy Johnson said - taped other teams. It was routine, according to him, but I never hear anyone looking to take Dallas' title away.

From what we've seen in baseball, cycling, football, and on and on, I pretty much believe that everyone is bending the rules somehow, someway. There's so much $$$$ at stake.
