On how they decide when to cancel flights on the edge cases when they aren't technically prohibited to use the airspace (FAA NOTAMs), and other carriers are still operating in the area.

Per your above, State Department can issue prohibitions with similar weight, but that only apply to domestic carriers. Seems strange to do that without looping in the FAA and their partner organizations.

Yes, and you indicated it's just state department. When announcements are made it appears to be airline carrier by airline carrier, so I assumed some private entity (be it airline, insurance, whoever) was making independent decisions. If it's a state department thing then as you say, insurance has nothing to do with it.
