Are there good live versions? Honest question. I sort of assumed that at this point Eddie's vocals kinda ruined any chance that a live version would top the studio version of any song.

Depending on how closely you want to listen to Ed, there are either a few good ones, or many. What makes this standout live is the crowd energy in the beginning (hit and miss) and the solo/full outro at the end (many great examples). The band plays it pretty tightly almost every time, and there are no annoying tambourines and salt shakers.

When was the last good one?
The number of the single is only a factor if the previous singles failed to establish audience interest. The first two singles here were some of the best performing of their post-Vitalogy career. So no, nobody is hesitating on it because it happens to be #3. If there's no response to the song then the disinterest is on the song, not the number.

I'm only interested if they do the fadeout live, gradually decreasing volume until they're just miming at playing their instruments

And as they do, they gradually slow to a pause and then just freeze. Eventually everyone leaves.

XM's Octane is pushing this single pretty hard despite the fact it doesn't fit the genre. Heard it 6 times in the last few days.

Good album track, not a single. It took me a while to 'learn' this song, which only indicates that it's not a strong single. There's too many crescendoes and parts stitched together, when a single needs to grab you on the very first listen.
