did rachel truly come back, or is her appearance just the visual representation of what toby truly hopes? or is it meant to be the happy ending of the book libby writes (since she does come back)?

How do you mean? It's a long scene that covers some ground.
It could have (maybe should have?) been the last scene. Everything after that is just sort of beside the point. The crux of what the show is about, the major themes and their resolutions, comes in that scene.
