LaFleur’s decision to kick a field goal on 4th down made me think of it.
The Seahawks throwing instead of running Marshawn is my pick for worst of all time.

Grady Little leaving Pedro Martinez in during Game 7 of the 2003 ALCS.
When Jeter hit his double with one out, it was 5-2 Red Sox with just 5 outs remaining, Pedro had already thrown 110 pitches. Mike Timlin and Alan Embree had a 0.00 ERA combined in 8 appearances in that series up to that point. This still astounds me.

Pulling Snell was god-tier fuck-up, but Carroll throwing on the goal line is definitely up there.

The Seahawks call is rightfully on anyone’s list, but feels different than other examples. The play could have worked. Also ball hits ground you have another play or two. Just blew up in their face on top of being a bad call. In the final seconds of the super bowl.
But the other stuff like pulling Snell, that was never going to work. LeFleur kicking the FG was never going to work.
Bad call coupled with poor execution vs. unforgivably bad call with no chance of working made by a complete idiot.

Whoever let #20 play the whole game for the Packers and then decided "hey let's go for for the field goal so that we now completely lose".

Pulling John Smoltz for Charlie Leidbrandt in the 91 series effectively handing one of the best pitched World Series games ever to Jack Morris. (Both pitchers were amazing).
Falcons trading for Michael Vick. San Diego used Atlanta’s pick for Ladanian Tomlinson, then used their own 1st pick of the 2nd round to draft some guy named Drew Brees.

The Fail Mary game(Seahawks-Packers 2012). At least it lead to the end of replacement refs.
The Fifth Down Game(Colorado-Missouri 1990). Colorado goes on to win a share of the national championship.

If we are talking officiating mistakes, I'm not sure there is a bigger one. This was objectively wrong.

That non PI call that the Rams rode to the Super Bowl was absolutely wrong.

1993 national championship between Michigan and UNC - Chris Webber calling a timeout that the team didn't have, and getting called for a technical.

Forgot about this. Also, a Georgetown player inexplicably throwing it right to a North Carolina player in the final moments of the 1982 National Championship.
