Seems like the twist should be that he's the killer the whole time, but they're flat out telling you that in the trailer, so maybe the twist is he's not actually the killer the whole time

the twist is whoever the killer is is trapping the arena.

no, the twist is that the movie is actually trapping the viewers who are actually the killers, and are actually all just ghosts the whole time, victims of the killers who are ghosts too
